WAIVER AND RELEASE OF LIABILITY – LITTLE STAR PLAYLAND

Acknowledging participation in activities at Little Star Playland, the undersigned, on their own behalf and on behalf of any executors, heirs, successors, and assigns, agrees as follows:

ASSUMPTION OF RISK

The undersigned knowingly and fully assumes all risks, known and unknown, associated with participation in all activities, including but not limited to slides, ball pits, arcade games, and other play equipment. I understand that injuries may range from minor (scratches, bruises, sprains) to major (eye injury, joint/back injury, concussion) to catastrophic (paralysis, death).

SUPERVISION RESPONSIBILITY

I understand that Little Star Playland employees are not responsible for supervising Participants. I am responsible for supervising any minors I bring into the facility.

EMERGENCY MEDICAL AUTHORIZATION

In the event of an injury or medical emergency involving my child(ren), I authorize Little Star Playland staff to provide basic first aid and/or seek emergency medical treatment as deemed necessary. I understand that I am responsible for all costs associated with such medical care. I release Little Star Playland from any liability arising from such medical treatment.

SAFETY RULES

I, and the children under my care, have read, understand, and agree to follow the safety rules and play guidelines posted at Little Star Playland. I acknowledge that these rules may change at any time.

ACKNOWLEDGMENT OF RULES COMPLIANCE

I agree to follow all posted safety rules and staff instructions while at the facility.

COVID-19 WARNING

I understand that exposure to COVID-19 in public spaces carries risks, including severe illness or death. Little Star Playland cannot guarantee safety or eliminate all risks. I voluntarily assume all risks associated with participation.

RELEASE & AGREEMENT NOT TO SUE

I, for myself, any minor Participant child, and our heirs, personal representatives, and assigns, release, discharge, and agree not to sue Little Star Playland, its managers, members, employees, or agents for any injury, illness, or death arising from participation in activities, including claims arising from negligence.

INDEMNIFICATION

I agree to indemnify and hold Little Star Playland harmless from any claims, actions, suits, damages, or expenses (including attorney's fees) arising from my or my minor Participant child's participation.

SECURITY CAMERAS

I understand that security cameras are installed for safety and may be reviewed in case of incidents or injury.

PHOTO & VIDEO RELEASE

I grant Little Star Playland permission to use photos, videos, or audio recordings of myself or my minor Participant child for legal purposes, including advertising, publicity, and website content, without compensation or additional consent.

FINANCIAL/PROPERTY DISCLAIMER

Little Star Playland is not responsible for any personal items that are lost, stolen, or damaged while on the premises.

SEVERABILITY

If any provision of this waiver is found invalid, illegal, or unenforceable, the remaining provisions shall remain in full force and effect.

MULTI-LOCATION APPLICABILITY

This waiver, when executed at any Little Star Playland location, applies equally to all other locations and the head office.

PARENT OR LEGAL GUARDIAN CERTIFICATION

I certify that I am the parent or legal guardian of the minor Participant(s) named below and have authority to sign on their behalf. I have read and fully understand this waiver, the risks of the activities, and voluntarily agree to all terms. This waiver has no expiration date.
